Welcome to The Hollies Family Surgery, Formby

Proposed merger with Chapel Lane Surgery - important information for all patients of The Hollies Family Surgery

09 May 2022

In early 2021, The Hollies Family Surgery arranged to work more closely with the partners at Chapel Lane Surgery, with both practices sharing responsibility for the delivery of services for The Hollies.

Since May 2021, both practices have continued to operate as 'stand alone' practices with their own reception and administrative staff but clinical staff have worked across both sites.

We are now looking at making this a more formal merger with the management and administrative functions of the two practices which will benefit patients, clinicians and support staff.

Please read the following document which explains this proposal in detail:

After you have read the above document, please click here to fill in a patient survey about the proposed merger. If you require a hard copy of the survey to fill out, please enquire at reception.
